The NJBIZ Health Care Heroes awards program was created in an effort to recognize excellence, promote innovation and honor the efforts of individuals and organizations making a significant impact on the quality of Health Care in New Jersey.

An independent panel of judges will evaluate the nominations and select several honorees in each of the categories. Honorees will be celebrated during an awards ceremony.

PLEASE NOTE: When you submit this form, your nominee will receive a Phase II application form. Please be sure to have correct contact information listed for your nominee.

NOMINATION FEE: $50.00 (non-refundable)

Categories of Recognition:

Health Care Professionals: Honors individuals (other than doctors or nurses) in health care whose job performance is considered exemplary by patients and peers.

Education: Honors any individual or organization in health education, that are making a difference in their community and in health care.

Innovation: Honors companies or individuals primarily responsible for a medical science discovery or for development of a new procedure, device, service, program or treatment that can save lives or improve the quality of life for a large number of people.

Nursing: Honors individuals from nursing whose job performance is considered exemplary by patients and peers.

Physicians: Honors physicians whose job performance is considered exemplary by patients and peers.

Advanced Practice Providers: Honors physician assistants, nurse practitioners and other licensed advanced clinicians whose exemplary care, leadership and patient impact elevate the standard of health care in New Jersey.

Public Health: Honors individuals (non-physicians and physicians) who have made or are making an extraordinary impact on the public health of their community.

Volunteer: Honors individuals who contributes time or expertise to patients or healthcare issues and whose volunteer efforts have made a significant positive impact on the community.

Workplace Wellness: Honors businesses for a company-wide wellness program, healthy work environment, ergonomics program, or other health-related activities benefiting employees.

Rehabilitation Therapy: Honors practices that have shown success in implementing or innovating a new product or service, or have helped advance the rehab therapy profession in general. Includes physical rehabilitation, speech & communication rehabilitation, cognitive or neuro-rehabilitation, etc.

Mental Health: Recognizes organizations that have shown outstanding achievements in improving mental health care services in the Garden State.

Ambulatory Health Care: Recognizes organizations providing outpatient care, including clinics, urgent care and other non-hospital settings that focus on patient treatment and wellness outside the inpatient setting.

Dental Care: Honors exemplary practices that offer general dentistry, pediatric dentistry, orthodontal services, periodontal services, oral surgery, etc. — the clinics responsible for diagnosis, treatment and advanced patient care.

Pharmaceutical Professional: Honors leaders involved in drug development, testing, regulation or patient care — including pharmacists, pharmaceutical scientists and other key contributors to bringing safe and effective medicines to patients.

Eligibility:

– Nominee’s company must have a physical office in NJ (may be for profit or not for profit). Nominees in the “individual” categories must also live and work in New Jersey.
– Nominations may be self-nominated or nominated by another person.
– Nominees may only be nominated in one of the categories listed above and on the form drop-down menu; however, the judges reserve the right to move an entry to a different category at their discretion.
– Nominees cannot be a member of the judges’ panel or member of the judges’ immediate family.
– Health Care Heroes honorees from the past two years must be nominated in a different category this year to be eligible.
– For categories marked “individual” (vs. organization), only one person can be nominated (this means groups of professionals are not eligible in that field). If more than one person is listed, the nomination will be disqualified.
– NJBIZ reserves the right to limit the number of honorees per company.
– If a nominee is chosen as an honoree, their participation is required at the event.
